Our Total and Total Plus plans are Medicare Advantage PPO plans for people with chronic health conditions like cardiovascular disease, chronic heart failure and diabetes. When you join, you get the benefits listed below. You also get coverage when you travel, a fitness program at no extra cost to you, and a wide choice of doctors and hospitals.

Both plans are available throughout Tennessee. Our Total plan is also available in Catoosa, Dade and Walker counties in North Georgia.
